What is Medicaid?

Medicaid is a safety net. One of its primary purposes is to fund the costs of nursing facilities and assisted living facilities. The average cost of assisted living in New Jersey is $6,000 per month or $72,000 per year, a huge sum for many. If you are eligible for Medicaid for seniors in NJ, these costs can be offset, ensuring you receive a high level of care even if your savings can’t pay for it all.

Who Is Eligible for Medicaid?

Eligibility for Medicaid in NJ depends on several factors. The Medicaid program is designed to help people pay for the often staggering costs of nursing homes and other long-term care options, but there’s a caveat. You must adhere to numerous rules and regulations to receive benefits.

To qualify for Medicaid nursing homes in New Jersey, you must:
• Be a legal U.S. citizen or qualified alien
• Be a resident of New Jersey
• Meet income and asset requirements
• Be 65 years of age, blind, or permanently disabled

Financial Requirements

Income

Income

NJ Medicaid eligibility also depends on an applicant’s income level. A long-term care applicant’s income may not exceed certain limits. As of 2025, these limits are $2,901 per month. If your income is higher than this, Senior Planning Services can help you set up a Qualified Income Trust (Miller Trust) so you can still qualify.
Assets

Assets

A NJ Medicaid long-term care applicant's assets must fall below specific thresholds to qualify. As of 2025, the Medicaid asset limits in New Jersey are $2,000 for single applicants, $3,000 for couples applying together, and up to $157,920 for the non-applicant spouse in cases where only one spouse is applying.

It's important to note that Medicaid enforces a five-year look-back period for financial transactions. During this time, any gifts or asset transfers may result in a penalty period of ineligibility for Medicaid coverage. This penalty corresponds to the total value of assets gifted or transferred during the look-back period. (There are some exceptions to the transfer rules which Medicaid may allow.)
Medical

Medical

Another one of NJ’s Medicaid eligibility requirements is a pre-admission screening (PAS) to determine the applicant’s medical status. A physical evaluation from a Medicaid nurse will ascertain whether you need assistance with activities of daily living (ADL), such as showering, dressing, eating, and toileting.

New Jersey Medicaid FAQ

What long-term care services does NJ Medicaid cover for seniors?
NJ Medicaid (NJ FamilyCare ABD) provides coverage for nursing home care, assisted living through managed long-term care, adult foster care, personal care at home, and some non-medical support to help seniors stay at home.
What are the income and asset limits for NJ long-term care Medicaid?
As of 2025, an individual’s monthly income must be no more than $2,901, and countable assets must be under $2,000.
What is NJ’s 5-year look-back rule, and how are penalties calculated?
NJ Medicaid examines all gifts and asset transfers made within the five years prior to a long-term care application. If disallowed, it forms a penalty period calculated by dividing the transfer amount by the state's daily divisor.
Can I get Medicaid in NJ if I own a home, and will the state place a lien on it?
Your primary residence is exempt from asset counting (up to specified equity limits) if a spouse or dependent lives there. Medicaid does not file a lien on your home while you’re alive. However, after your death, if no exempt heirs remain, the state may recover Medicaid costs from your estate via a lien. A Senior Planning Services consultant can clarify exactly what your Medicaid rights are.
What medical assessment is needed to qualify for Medicaid in New Jersey?
Applicants must undergo a Pre-Admission Screening (PAS) to demonstrate a nursing facility level of care, meaning they require assistance with activities of daily living (like bathing, dressing, or eating).
